We will contact you a few days prior to your scheduled appointment to confirm the appointment date and time, and to pre-register your demographic information. When you check in for your appointment, we require a copy of your insurance card(s) and a valid ID. 

It is our goal to follow our time schedule as closely as possible, except when an emergency arises that may cause a delay. We ask that new patients arrive at least 30 minutes prior to the scheduled appointment time and established patients 15 minutes prior. Please ensure that all required forms are as complete and accurate as possible before arrival.
